How many times can All Will Be One be triggered in a single action

Say I have All Will Be One, Urtet, Remnant of Memnarch, and three myr tokens. If I activate Urtet, Remnant of Memnarch's ability and give all the myrs three +1/+1 counters, does All Will Be One trigger only once dealing 12 damage to a single target; or does it trigger 4 times and allowing me to target up to 4 targets?

It will trigger 4 times allowing you to choose different targets. It triggers for each different permanent/player you’re giving counters to.
