I have never seen anyone make a deck like this! Title is accurate! Awesome brew
It seems it’s been working well with just the 11 or so moonfolk you’ve been running, but you ever think of running some changelings? On a quick search I think there’s 4-5. Something like Shapesharer Universal Automaton or Turtleshell Changeling. The only reason to use them would be if your moon-folk count isn’t assisting with your commander enough.
You ever think of some snow-covered islands to work in tandem with Extraplanar Lens? That way if you play against another blue deck you don’t double their mana as well.

Turtleshell Changeling (replace with 1 Calcite Snapper and 1 Horned Turtle) and Tortoise Formation (replace with 2 Spell Pierce) plus Meandering Towershell (replace with 2 Thriving Turtle) and for better blocking Freed from the Real (replace with 1 Essence Scatter) annnnnnnnd to keep your Azor's Elocutors on the battlefield Darksteel Plate (replace with 2 Island) Also my strat for this deck is: block get out your Azor's Elocutors put the Darksteel Plate on them then block with your turtles so you dont take damage (If you something like a Lava Axe use a counter spell) untell you can automatically win the game. FLAWLESS!!!

No it would not.
This is where everyone's favorite part of the rules kicks in - layers!
The way layers work is that the game defines specific groups of conditions that change the aspects of a card. The lower the number, the earlier it applies. We're looking exclusively at layer 7 here, which deals with power and toughness and is split into 5 more layers itself.
The layers apply in the following order:
1. Define P/T
2. Set P/T
3. Modify P/T without setting it
4. Counters (e.g. +1/+1)
5. P/T switching
So let's look at the situation you have. With the Turtleshell Changeling and Plated Sliver on the field, the changeling will become a 1/5. If you use its ability, it becomes a 5/1. It doesn't gain any extra from the Sliver because it already has. This is because of rule 613.5, which reads:
If an effect should be applied in different layers and/or sublayers, the parts of the effect each apply in their appropriate ones. If an effect starts to apply in one layer and/or sublayer, it will continue to be applied to the same set of objects in each other applicable layer and/or sublayer, even if the ability generating the effect is removed during this process.
This also explains why the Changeling doesn't become a 4/2 when it switches its P/T.

Looking at the deck again I think Sword of Body and Mind doesn't fit too well here.
I think you should remove these creatures:
Cursecatcher is way too specific.
Deep-Sea Kraken is slow.
Deepchannel Mentor costs a lot for its ability. Personally I think you have enough better aversion.
Mothdust Changeling takes a lot of help to make it good.
Silvergill Douser doesn't seem powerful enough.
Turtleshell Changeling is...meh. In EDH, getting in that possible 2 extra damage is not really worth it.
